CRAFFT - 6-item substance abuse screening instrument for adolescents
|
The CRAFFT is a behavioral health screening tool for use with children under the age of 21 and is recommended by the American Academy of Pediatrics' Committee on Substance Abuse. It consists of a series of 6 questions developed to screen adolescents for high risk alcohol and other drug use disorders simultaneously. It is a short, effective screening tool meant to assess whether a longer conversation about the context of use, frequency, and other risks and consequences of alcohol and other drug use is warranted. The questions should be asked exactly as written. A pocket card with the CRAFFT questions is available for clinical use. |

The DAST has “exhibited valid psychometric properties” and has been found to be “a sensitive screening instrument for the abuse of drugs other than alcohol". |

Hearing Handicap Inventory for the Elderly - Screening Version
|
The screening version of the Hearing Handicap Inventory for the Elderly (HHIE-S) is a 5-minute, 10-item questionnaire developed to assess how the individual perceives the social and emotional effects of hearing loss. Persons who perceive their hearing loss to be a problem are more likely to have further testing and accept the need for a hearing aid. The HHIE-S was designed to be used with non-institutionalized older adults in a variety of clinical and community settings. Audiologic referral is recommended for individuals scoring 10 or higher on the inventory. |
